Background: Focal task-specific dystonia is a form of isolated focal dystonia that occurs during the performance of a specific skilled motor task. The occurrence of oromandibular dystonia (OMD) specifically in association with the recitation of Quranic verses have been rarely reported in the literature, in non-native Arabic-speaking patients. This case series describe a rare type of focal task-specific dystonia that occurs exclusively by reciting Quran in native Arabic-speaking patients, which has never been reported, to the best of our knowledge. Methods: In this case series, we identified five patients with new-onset OMD that was exclusively induced by reciting Quran. Cases were evaluated in our Movement Disorders outpatient clinic at Ibn Sina hospital; the main tertiary neurology center in Kuwait, between 2015 and 2023. Results: Five cases (3 males, 2 females) were identified in this study. Mean age of onset of the symptoms was 52.3 ± 4.1 years, while the median duration of the symptoms prior to diagnosis was 3 years. All patients were native Arab-speaking, with no previous history of other types of dystonia. No identifiable risk factors could be obtained including exposure to dopamine blocking agents or antipsychotics, or history of oral or dental surgery. Patients underwent a full clinical, laboratory, and radiological evaluation. All patients had OMD dystonia in varying forms and severity, while two patients had additional spasmodic dysphonia/ blepharospasm on progressive recitation. Most patients had minimal improvement with combination of oral medications and speech therapy. Four patients received botulinum toxin injections with better results. Discussion: The mental and physical stress in attempting to recite the Quranic verses could have contributed to the development of OMD. Moreover, the increased demand on the muscles of the jaw, lips, and tongue during recitation can trigger the dystonic symptoms. Highlights: OMD exclusively during Quran recitation is a rare phenomenon, and expands the spectrum of task-specific focal dystonia described in the literature. It was found to be distressing to the patients and a challenge to treat. Prompt recognition could minimize unnecessary testing and procedures, and facilitate earlier treatment.

Background: In cases of atopy, IgE antibodies are formed in reaction to certain environmental factors like house dust mites, pollen, or even food allergens. Some studies have shown an association between atopic diseases and keratoconus. This study aims to determine the prevalence of atopy and its associated factors among keratoconus patients. Materials and Methods: A cross-sectional study was conducted among the Jazan population attending ophthalmology clinics regardless of their sex. Data collection started in August 2020 and finished in mid-June 2021. The sample was selected using the convenience sampling technique, and the total sample size was 83 participants. Data were collected via a questionnaire-based interview that contained 46 questions in the Arabic language. The data collected were analyzed using SPSS software. Results: Family history is the leading predisposing factor for keratoconus (P < 0.001). Also, eye rubbing was a significant (P < 0.015) trigger for keratoconus. However, other factors, including demographic factors such as gender and atopic conditions, did not have any significant association. Conclusion: A combination of genetic and environmental factors is the leading cause of the development of keratoconus; however, family history is the most influential factor, followed by eye rubbing.

Wünderlich's syndrome, or spontaneous renal haemorrhages, are rare, atraumatic, intraparenchymal and perirenal haemorrhages, which are difficult to diagnose and can be potentially fatal.Patients who are dialysis-dependent are at an increased risk of bleeding, due to an association between uraemia and platelet dysfunction; for this reason, the use of double antiplatelets is avoided in this cohort. Case studies exist demonstrating spontaneous bleeds in these patients.Fish oil is used increasingly within medicine: however, it is known to interfere with platelet aggregation, therefore, theoretically increasing the tendency to bleed. The topic remains under debate, with systemic reviews refuting a correlation between intraoperative and postoperative bleeding and fish oil consumption. There is, however, an absence of literature on the adverse effects of fish oil when taken in large quantities.This case study explores the case of a patient who had a spontaneous renal bleed following a large, self-medicated dose of fish oil.

Introduction: Recently, the cases of arteriosclerotic cardiovascular disease (ASCVD) have been increasing in the rural areas of southwest Saudi Arabia. Therefore, this study sought to consider the relevant contributing factors, such as traditional dietary habits and a diet rich in cholesterol and fat, few opportunities for disease screening, false beliefs about chronic disease management medications, delays in diagnosis, and the great distances required for travel to cardiac specialist centers. Methods: A cross-sectional study was conducted at 12 primary health care (PHC) centers, using the ASCVD tool. The total required sample was determined to be 440 participants after calculating based on the total population of Al-Harja. Systematic random sampling was performed using a list of PHC centers and fixed intervals. The study's outcome is ASCVD risk score divided into four categories: low risk <5%, borderline risk 5%-7.5%, intermediate risk >7.5%-20%, and high risk >20%. Data were collected through interview questionnaires, measurements, and laboratory sample investigations. Result: It was revealed that 16.59% of the participants were classified in the high-risk category for having ASCVD in the next 10 years. There was a significant association between the ASCVD risk score and obesity (P = 0.03). Prior diagnoses for either hypertension or diabetes mellitus (P = 0.00) were significantly associated with an increased ASCVD risk score. Conclusion: An alarmingly high prevalence of high-risk ASCVD scores was seen among male participants, with 54 (22.78%) in the high-risk category, while 19 female participants (9.36%) were in the same category. Therefore, we can conclude that male participants are two times as likely to have a high ASCVD risk than female participants.

AIM OF THE STUDY: To assess the awareness and practice of Immediate Dentures among the dentists who work in the private clinics in Khartoum state, Sudan Materials and methods: A descriptive cross-sectional questionnaire-based study was conducted among the dental medical officers, registrars, specialists, and consultants who are working in private clinics and centers in Khartoum state and registered the private medical institutions in Sudan. The sample population was 664, using the stratified sampling technique and simple randomization within the stratum, 265 dentists were asked to participate in the study voluntarily A self-administered questionnaire consisted of 32 closed questions and 3 sections, section (1) involved the participant's data, section (2) contained the participant's knowledge, and section (3) the participant's practice was distributed. The participant's knowledge and practice scores were calculated as percentages achieved by dividing the number of accurate answers of the participants by the total number of questions and categorized as Good (66.6-100%), Average (33.3-66.6%), and Poor (less than 33.3%) Data was collected and analyzed using a statistical package of Social Science (SPSS) computer software version 22. Chi-square was used for statistical analysis Results: The response rate was 100%, with nearly equal distribution between males (135, 50.9%) and females (130, 49.1%). The age range is 24-39 years, with a mean of 31.65 ± 7.52 years. The majority of the participants (172, 64.9%) were freshly graduated and young practitioners, with the majority of them having experience ranging from 1 to 6 years. The result of the study revealed that 65.7% of the respondents have average knowledge about immediate denture while 8.7% expressed poor knowledge and only 25.7% had excellent knowledge On the other hand, the majority of the participants (200,75.5%) have never constructed an immediate denture, while only 21.1% have a history of constructing 1-5 immediate dentures, 3% constructed more than 5-10 immediate dentures and only 0.4% constructed more than 10 immediate dentures The lack of knowledge and experience was the main barrier preventing the use of immediate dentures, and the mainstream of the participants (118, 44.5%), evaluated their knowledge about the immediate denture as poor Using the Chi-square test, the correlation between the participants' knowledge and their gender, years of experience, and qualification was found not significant (p -values of 0.46, 027, and 0.66, respectively) while a significant correlation was found between the participants' self-evaluation of their knowledge and their knowledge score with a p-value of 0.000* Conclusion: Within the limitations of this study, the dental practitioners have average knowledge about the immediate denture, but the practice of the immediate denture is still very limited and requires enhancement Clinical significance: Immediate dentures have a significant role in maintaining the patient's esthetics, phonetics, masticatory efficiency, self-esteem, and quality of life. Industrial and agricultural advances have led to global issues such as contamination of water sources and lack of access to clean water. Wastewater from petroleum refineries must be subjected to treatment as it poses a significant environmental threat. The present research aimed to reduce the level of chemical oxygen demand (COD) of an effluent from Bijee petroleum refinery plant, Iraq, using solar photo-electro-Fenton (SPEF) process operated in a batch recycle model. The electrochemical reactor used in the present research was of a tubular design with an anode composed of porous graphite rod and a concentric cylindrical cathode made of the same material. The impacts of operating parameters such as current density (10-50 mA/cm2), Fe2+ concentration (0.2-0.8 mM), NaCl addition (0-1 g/L), and time (30-90 min) on the COD removal efficiency were explored based on the response surface methodology (RSM). Results showed that the impact of Fe2+ concentration was most prominent, with an effective contribution of 47.7%, followed by current density, with a contribution of 18.26%, and the addition of NaCl, with a contribution of 11.20%. COD removal was found to increase with an increase in current density, Fe2+ concentration, NaCl addition, and time, respectively, while energy consumption was found to increase significantly with an increase in current density and a decrease in Fe2+ concentration, respectively. The optimum conditions were observed to be an initial pH of 3, current density of 10 mA/cm2, Fe2+ concentration of 0.8 mM, NaCl addition of 0.747 g/L, and a duration of 87 min, upon which 93.20% COD removal efficiency was achieved, with an energy consumption of 15.97 kWh/kg COD.

Purpose: Coronavirus 2019 (COVID-19) had major social, medical, and economic impacts globally. The study aims to develop a deep-learning model that can predict the severity of COVID-19 in patients based on CT images of their lungs. Methods: COVID-19 causes lung infections, and qRT-PCR is an essential tool used to detect virus infection. However, qRT-PCR is inadequate for detecting the severity of the disease and the extent to which it affects the lung. In this paper, we aim to determine the severity level of COVID-19 by studying lung CT scans of people diagnosed with the virus. Results: We used images from King Abdullah University Hospital in Jordan; we collected our dataset from 875 cases with 2205 CT images. A radiologist classified the images into four levels of severity: normal, mild, moderate, and severe. We used various deep-learning algorithms to predict the severity of lung diseases. The results show that the best deep-learning algorithm used is Resnet101, with an accuracy score of 99.5% and a data loss rate of 0.03%. Conclusion: The proposed model assisted in diagnosing and treating COVID-19 patients and helped improve patient outcomes.

Aim: Correct orientation of the occlusal plane plays a vital role in achieving the perfect occlusal balance and function of complete dentures. This study aimed to evaluate the most reliable posterior reference point of the ala-tragus line (ATL) concerning occlusal plane (OP) in a sample of the dentate Sudanese population. Materials and Methods: A total of 150 subjects with healthy and well-aligned permanent teeth were randomly selected. Right lateral profile photographs were taken with subjects having a fox plane placed intra-orally, contacting the occlusal plane. Reference points corresponding to inferior, middle, and superior borders of the tragus and inferior border of the ala of the nose were marked on photographs. The angles between the lines were measured using the Auto-CAD software program, and the most parallel relationship was determined. Descriptive statistics in terms of means and standard deviations were presented. Independent t-test and one-way ANOVA tests were used to compare as appropriate. A p-value < 0.05 was considered significant. Results: The mean angle formed by the OP and ATL was 8.5±3.69º for the superior level, 4.68±3.13º for the middle line, and 2.89±2.57º for the inferior line. A significant difference was found between the means of the three angles (p< 0.001), while no significant difference (p> 0.05) was found between both genders regarding the measured angles. Conclusions: The line joining the inferior border of the ala of the nose with the inferior border of the tragus of the ear was the most reliable line in terms of parallelism to determine the occlusal plane orientation.


Antecedentes: La orientación correcta del plano oclusal juega un papel vital para lograr el equilibrio oclusal perfecto y la función de las prótesis completas. Este estudio tuvo como objetivo evaluar el punto de referencia posterior más confiable de la línea ala-trago (ATL) con respecto al plano oclusal (OP) en una muestra de la población dentada de Sudán. Materiales y Métodos: Se seleccionaron aleatoriamente un total de 150 sujetos con dientes permanentes sanos y bien alineados. Se tomaron fotografías de perfil lateral derecho de sujetos a los que se les colocó un plano de zorro intraoralmente, en contacto con el plano oclusal. En las fotografías se marcaron los puntos de referencia correspondientes a los bordes inferior, medio y superior del trago y al borde inferior del ala de la nariz. Los ángulos entre las líneas se midieron utilizando el programa de software Auto-CAD y se determinó la relación más paralela. Se presentaron estadísticas descriptivas en términos de medias y desviaciones estándar. Se utilizaron prueba-t independiente y prueba ANOVA unidireccional para las comparaciones, según correspondiera. Se consideró significativo un valor de p<0,05. Resultados: El ángulo medio formado por OP y ATL fue de 8,5±3,69º para el nivel superior, 4,68±3,13º para la línea media y 2,89±2,57º para la línea inferior. Se encontró una diferencia significativa entre las medias de los tres ángulos (p< 0,001), mientras que no se encontró diferencia significativa (p>0,05) entre ambos sexos con respecto a los ángulos medidos. Conclusión: La línea que une el borde inferior del ala de la nariz con el borde inferior del trago de la oreja fue la línea más confiable en términos de paralelismo para determinar la orientación del plano oclusal.

Background: Spiral drawing is an important test in monitoring essential tremor (ET). With the rise of telemedicine amid the coronavirus disease 2019 pandemic, a contactless tool for monitoring tremors was required. We aimed to assess the validity of a novel smartphone technology using a video-based social media platform for rapid and objective monitoring of ET. Methods: A prospective pilot study evaluated patients with ET in 2 clinic visits. Videos of tremors were recorded using a publicly available Instagram filter and were visually compared with spirals drawn by the patients. The level of agreement among the raters was evaluated. Results: A total of 12 patients with ET were recruited. A consensus between both raters was achieved for 11 patients (91.6%) for both spirals and videos with good interrater agreement (κ value, 0.755 ± 0.332). Conclusion: This novel method was found to be valid and easy to use in measuring ET in real-world settings. Further research in a larger cohort is needed to suggest its use as a home-based or clinic-based monitoring tool.

Crop diversification with cover crops could deliver a wide range of agroecosystem services including water conservation, nutrient cycling, biodiversity, and crop productivity as well as reducing the negative environmental footprint of conventional fallows. However, the potential competition of cover crops and subsequent cash crops on plant available water and soil mineral nitrogen (N) has limited the adoption of cover cropping for fallow replacement in water-limited environments. This article provides datasets for understanding the multifunctional role of cover crops as an alternative paradigm to conventional fallow in water-limited environments. The dataset is divided into four components comprising measured cover crop parameters (21 variables, n = 144), soil water (4 variables, n = 2,159), soil mineral N (4 variables, n = 1440), and site characteristics (8 variables). The datasets consist of crop resource quantity (biomass, N uptake, δ¹5N (‰), fixed shoot N, and water use), resource quality (C/N ratio, lignin, acid detergent fibre, and N concentration), and soil status (plant available water and mineral N) at field scale. The data supports the framework of the continuous development of alternative innovative cropping systems that have the potential to increase and maintain crop yield while minimizing the adverse effects of conventional fallowing in the context of sustainable intensification. The datasets are associated with the original research article published in Agriculture, Ecosystem, and Environment entitled "Fallow replacement cover crops impact soil water and nitrogen dynamics in a semi-arid sub-tropical environment" as Garba et al. [1].

BACKGROUND: Television medical dramas enjoy great popularity among the general public, and can be a source of information and misinformation about medical disorders. Nervous system disorders have always received attention in popular media, yet no studies have been performed to analyze their depiction and accuracy, to our knowledge. OBJECTIVE: To investigate the representation of neurological and neurosurgical diseases in Grey's Anatomy, one of most popular and longest running primetime medical melodramas in American television. METHODS: We performed a quantitative and qualitative content analysis of depictions of nervous system disorders in all 18 seasons of Grey's Anatomy, in addition to investigating the medical accuracy and the global quality of the portrayed medical content. RESULTS: A total of 285 depictions were identified in 314 out of 400 episodes (78.5%) of Grey's Anatomy. Most of portrayed characters were males (59.2%), Caucasians (72.6%), and adults (76.4%), with a mean age of 32.6 ± 19.7 years for those mentioned. Most of the portrayals were of acute nature (65.5%), with neurosurgical diseases constituting the majority; 85.6%, while 31.6% depicted neurological diseases, and 19.3% had both. The most common clinical presentations were altered level of consciousness (16.5%) and seizures (14.4%). Traumatic brain injury was the most common portrayed diagnosis (39.3%), followed by brain tumors (13.7%), stroke (8.4%), and spinal injury (8.1%). Management was surgical for the majority of cases (79.6%), while only 17.9% received pharmacotherapy. The prognosis of portrayed cases was generally favorable, as 79.5% showed good/full recovery, while mortality rate was 18.6%. Forty cases (14.0%) were depictions of rare to very rare diseases. As regards to medical accuracy, 74.3% of depictions were deemed accurate within reason, with a mean GQS score of 4.38 ± 1.4. Four clinical trials involving the nervous system have been depicted throughout the show. CONCLUSIONS: Neurosurgical and neurological diseases were portrayed in most episodes of Grey's Anatomy, with a good quality of its scientific content within reason. This study demonstrated that Grey's Anatomy had a good amount of medical information that could be able to improve the public perception of the specialty. However, there is still a room for significant improvement to acknowledge certain inaccuracies and misrepresentations in future episodes.

Background: Assessment of the quality of life (QoL) among healthcare workers (HCWs) is vital for better healthcare and is an essential indicator for competent health service delivery. Since the coronavirus disease 2019 (COVID-19) pandemic strike, the frontline position of HCWs subjected them to tremendous mental and psychological burden with a high risk of virus acquisition. Aim: This study evaluated the QoL and its influencing factors among HCWs residing in the Arab countries. Methods: This was a cross-sectional study using a self-administered online questionnaire based on the World Health Organization QoL-BREF instrument with additional questions related to COVID-19. The study was conducted in three different languages (Arabic, English, and French) across 19 Arab countries between February 22 and March 24, 2022. Results: A total of 3,170 HCWs were included in the survey. The majority were females (75.3%), aged 18-40 years (76.4%), urban residents (90.4%), married (54.5%), and were living in middle-income countries (72.0%). The mean scores of general health and general QoL were 3.7 ± 1.0 and 3.7 ± 0.9, respectively. Those who attained average physical, psychological, social, and environmental QoL were 40.8, 15.4, 26.2, and 22.3%, respectively. The income per capita and country income affected the mean scores of all QoL domains. Previous COVID-19 infection, having relatives who died of COVID-19, and being vaccinated against COVID-19 significantly affected the mean scores of different domains. Conclusion: A large proportion of the Arab HCWs evaluated in this study had an overall poor QoL. More attention should be directed to this vulnerable group to ensure their productivity and service provision.

Coronavirus disease (COVID-19) booster doses decrease infection transmission and disease severity. This study aimed to assess the acceptance of COVID-19 vaccine booster doses in low, middle, and high-income countries of the East Mediterranean Region (EMR) and its determinants using the health belief model (HBM). In addition, we aimed to identify the causes of booster dose rejection and the main source of information about vaccination. Using the snowball and convince sampling technique, a bilingual, self-administered, anonymous questionnaire was used to collect the data from 14 EMR countries through different social media platforms. Logistic regression analysis was used to estimate the key determinants that predict vaccination acceptance among respondents. Overall, 2327 participants responded to the questionnaire. In total, 1468 received compulsory doses of vaccination. Of them, 739 (50.3%) received booster doses and 387 (26.4%) were willing to get the COVID-19 vaccine booster doses. Vaccine booster dose acceptance rates in low, middle, and high-income countries were 73.4%, 67.9%, and 83.0%, respectively (p < 0.001). Participants who reported reliance on information about the COVID-19 vaccination from the Ministry of Health websites were more willing to accept booster doses (79.3% vs. 66.6%, p < 0.001). The leading causes behind booster dose rejection were the beliefs that booster doses have no benefit (48.35%) and have severe side effects (25.6%). Determinants of booster dose acceptance were age (odds ratio (OR) = 1.02, 95% confidence interval (CI): 1.01-1.03, p = 0.002), information provided by the Ministry of Health (OR = 3.40, 95% CI: 1.79-6.49, p = 0.015), perceived susceptibility to COVID-19 infection (OR = 1.88, 95% CI: 1.21-2.93, p = 0.005), perceived severity of COVID-19 (OR = 2.08, 95% CI: 137-3.16, p = 0.001), and perceived risk of side effects (OR = 0.25, 95% CI: 0.19-0.34, p < 0.001). Booster dose acceptance in EMR is relatively high. Interventions based on HBM may provide useful directions for policymakers to enhance the population's acceptance of booster vaccination.

The tropical peatland ecosystems of Indonesia provide direct economic benefits to local communities and act to maintain local weather patterns. The impact of burning tropical peat swamp forests of land clearing for palm oil plantations can have significant consequences on the change in the characteristics of peat soil. The aim of this study was to determine the physical, chemical, and biological properties of peat soils by field and laboratory testing and analysis to understand changes in the nature and characteristics of peatlands at four locations in the Pelalawan Regency of Riau Province. The results showed that the effect of burning peat swamp forests can lead to a change in the physical, chemical, and biological properties of the peat soils. Soil permeability and the soil microbial population can significantly decrease with increasing fire severity. The effect of different fire severities on the characteristics of peat soil is verified to contribute to advanced management of the tropical peatland in the future.

BACKGROUND: Lumbar puncture (LP) is a common and relatively safe neurological procedure. It can be complicated by post-dural puncture headache (PDPH) after both diagnostic and therapeutic procedures. The aim of this study is to identify the incidence, risk factors and clinical characterization of PDPH in the inpatient setting of the main tertiary neurology hospital in Kuwait. METHODS: We conducted a prospective observational cohort study that included patients who were admitted to neurology department at Ibn Sina hospital, Kuwait, from January 1, 2019 to December 31, 2020, on whom, LP was performed for diagnostic and/or therapeutic reasons. Multivariate logistic regression analysis was performed to evaluate the association between PDPH and different clinical parameters. RESULTS: A total of 285 patients were included; 225 females (78.9%), mean age of 32.9 ± 11.7 years. PDPH was reported by 84 patients (29.5%), with mean headache onset of 1.7 ± 0.8 days, and mean duration of 2.4 ± 2.1 days. The commonest headache type was dull aching in 49 patients (58.3%). Headache severity was mild to moderate in 64 patients (76.2%), with mean NRS of 4.1 ± 0.9. Most PDPH (99.3%) resolved with conservative medical management, with only 2 patients (0.7%) requiring epidural blood patch. In multivariate logistic regression model, there was a statistically significant correlation between development of PDPH and young age (p = 0.001), female gender (p = 0 .001), low BMI (p < 0 .001), pre-LP headache (p = 0.001), history of previous PDPH (p = 0.001), and number of LP attempts (p < 0.001). PDPH was statistically significantly higher in patients with optic neuritis (p = 0.009), and cerebral venous thrombosis (p = 0.007), and lower in patients with peripheral neuropathy (p = 0.011) and spinal muscular atrophy (p = 0.042). CONCLUSIONS: Findings from clinical practice in the main tertiary neurology hospital in Kuwait were in line with literature findings. Younger age, female gender, lower BMI, pre-procedural headache, previous history of PDPH, and number of LP attempts were found to be independent risk factors for developing PDPH. To our knowledge, this study represents the first comprehensive description of PDPH in a population from the Arabian Gulf Region.

Nosocomial infections have serious effects on health conditions in humans and animals. The present study aimed to investigate the pathogenesis of Enterobacter cloacae post intraperitoneal inoculation in rabbits to investigate the immunological and possible pathological effects. A total of 42 rabbits were randomly divided into two equal groups (n=21). The first group was inoculated with 3×108 CFU/ml of the virulent isolate of E. cloacae intraperitoneally (IP), while the second group was injected IP with phosphate buffer saline and considered a control negative group. The animals were sacrificed at different time post-infection at 48/72 h, and at day 7 post-bacterial inoculation. The results revealed a significant increase in the concentration of TNF-α, especially in the infected groups. In addition, there were different pathological lesions in different organs of animals, mainly in the infected groups, which represents by vascular congestion and edema with polymorphoneutrophiles infiltration in the lungs, kidneys, and heart. This study is considered the first trial which aimed to observe the pathological changes of E. cloacae in vital organs in rabbits.
